Case Study-Health Services Coordination

A 56 year old woman visited the ED frequently for non-emergency reasons, such as a stomach ache. The patient visited the ED 14 times within a 4-month period. A Care Coordinator met with her in the ED and the patient agreed to program services.
The Care Coordinator established a relationship with the patient and arranged a new PCP, medication support, and a therapist. The Care Coordinator obtained the assistance of a home aide. The patient has followed through on her appointments to-date.
Since development of her care plan, the patient has returned to the ED once.
